A Continuum of Care
For over 65 years, we have been providing compassionate care to our seniors while adhering to the highest standards of excellence. Grace Village offers accommodations adapted to the daily needs of residents according to their degree of autonomy. Our nurses provide a wide range of geriatric and cognitive care, physical and medical support, as well as specialized care for memory disorders and dementia.
Our medical staff and care team maintain a close relationship with regional health care experts, as well as with the renowned specialized medical resources of the Centre Intégré Universitaire de Santé et de Services Sociaux de l’Estrie – Centre Hospitalier Universitaire de Sherbrooke (CIUSSSE-CHUS), and the Institut Universitaire de Gériatrie de Sherbrooke (IUGS), located just minutes from Grace Village.

Assessing the Level of Care
Our Director of Care, in collaboration with the nursing staff and social workers at CIUSSSE-CHUS, assesses users’ needs based on their functional autonomy (ISO-SMAF profile), determining the level of care required. The cost of services is then determined on the basis of the ISO-SMAF profile and the four basic categories described below.

- Independent or autonomous living
- Semi-autonomous living (SMAF 1-4)
- Basic Nursing Care (SMAF 5-9)
- Advanced Nursing Care (SMAF 10-14)
- Convalescence, respite, palliative and end-of-life care
Live independently during retirement while offering:
- The possibility of taking advantage of medical services (nursing and medical care)
- The choice to participate in all available activities
Receive at-home assistance and services as needed.
This formula is designed for seniors who are able to maintain an essentially independent lifestyle, but who need access to medical support and/or basic support services.
For seniors who are essentially mobile and independent for most basic activities but require daily medical care from an attendant and/or nurse.
Intended for seniors with increasing needs for mobility assistance and daily medical services due to increasing disability. Including specialized care for residents with cognitive impairment, dementia or Alzheimer’s.
Intended for seniors according to their needs.
